Preceded in death by grandfather, Steve Toms, survivors include his parents, Bob and Lisa Polk, grandparents, Geneva Toms and Joe and Jan Doyon; brother, Marc (Naomi) Polk, nephews, Jameson and Ronan; uncles, Steve (Paula) Doyon and Dan (Jeanette) Polk and cousins Kristina and Andrea.
